Wellness Extended Office Visit (Maintenance Care) = $40 + $15 ea. additional 15 minutes

Initial Evaluation = $250
Re-evaluations = $200
Regular Office Visit = $75
Extended Office Visit = $140 + $65 ea. additional 15 minutes
 
Time of Service Discounts:

Discount Program (PCD) - Purchase an individual discount card for $30 or a family card for $45 and take advantage of the following packages.
